Please contact the Board Administrator at 502-782-0722 if you have any questions about renewal. Renewals are due no later than June 30th. Failure to renew by this date will add an additional $125.00 late fee. If renewal is not complete by December 31st, the license will be canceled on January 1st.
If you are not sure if your continuing education was reported, please call the Board Administrator, as a $300 penalty will be added if education has not been completed and reported on or before the deadline May 31st.
